COVID-19 Vaccine Update 

2/15/21 As a Healthmart Federal Partner, our COVID Vaccine protocol must adhere to the following procedure.

1. Visit healthmartcovidvaccine.com

2. Register as a new user. Each individual will need their own unique email.

3. Complete the screening questionnaire and assessment.  (This is the current stopping point until we receive the vaccine.)

4. Select appointment time and date.

5. Generate a voucher.

 

We do have staff available to help with this process or if you need help setting up an email account.

If you have your name on our list we will be contacting you begin this process.
